Daybreak / Substance Abuse and Medication Management

Daybreak is a pilot project in cooperation with White Oaks and the Department of Human Services, Office of Alcohol and Substance Abuse. The pilot is specifically geared toward seniors and their unique issues.

Daybreak offers counseling, medication review and social services. The program works not only with the senior, but also caregivers and physicians. We have a registered nurse, a certified substance abuse counselor, a licensed practical nurse and an outreach worker on staff to serve the needs of our client and assisting them in a harm reduction process by providing them the education and support they need.
