If you are a person 60 years of age or older,
or a Caregiver taking care of someone who is at least 60 or
older, who is having difficulties with everyday tasks around the
house, such as preparing meals, keeping the house clean,
bathing, grooming, or finding a ride to doctors appointments,
then you may be in need of Case Management Services. Case
Management Services are designed to assist seniors to continue
to live in the community for as long as possible by providing
them with the necessary assistance to do so.
If you feel that you could benefit from these
services, or know someone who could benefit from these services,
then contact the Central Illinois Agency on Aging, so that we
can assist you in finding the help that you need to remain in
your home living as independently as possible. The Central
Illinois Agency on Aging can refer you to one of three Case
Coordination Units; that provide Case Management services to our
six county service area of Fulton, Peoria, Marshall, Stark,
Tazewell, and Woodford Counties, and a case manager will come to
your home to determine what needs that you have, and with your
help develop a plan to address those needs. For more information
